This is a bruiser Grux with lots of +HP, power and on-hit

Crest- Warrior's > Fenix - Still likely the best crest. Against HEAVY CC you could consider liberator in marksman or Brutallax
Kingsbane - Crazy on-hit procs which are strong early and also have some scaling.
Bonesaw - Slow + HP + DMG/HP
Basilisk - Armor Shred + HP
Citadel - more armor shred + HP + armor
Flex - This has changed a lot several good flex options below.

Enemy team has ~2+ CC - Legacy (or absolution, up to preference)
Enemy team has 2 Phys armor tanks (and it is important that you and not the ADC need to kill them) - Breach
Enemy team has high HP tanks (not phys armor) - Mutilator

UPDATE - 0.4.3 Alternate
Alternate build for frontline tanky grux. This build BEGS the enemy to focus you so it procs all 3 items and you become an unstoppable force with tons of power, speed, and lifesteal.
