Your regular rotation has 0.5+0.3+0.7 ratios. We can add another 0.65 for trades or 0.77 for all-ins with the On-hits. That's 2.15 ratio. With just the crest and first two items Magic damage, we have a Q+RMB+E+Q(back) trade for 312 damage that is completely safe and on a very short cooldown (not including the base damage of these abilities which would be pushing 500 damage. at that point.

Cres + Oathkeeper give you an additional 150 + 77% scaling ability. Cres gives you early game mana regen, it's a cheaper path to the 50 damage burst and the 15% is more valuable when cd's are higher.
These two items are free to fit in anywhere after Oathkeeper depending on the situation. Healing reduc, Ignoring MR, and Tenacity
Because of Cres + Oath, we want to take advantage of Countess's already large Magic ratios by getting as much Magical Damage as possible
Don't save this for a fight. It's cooldown is fast enough that it will be available most times you need it, but the stacks really can make that difference of a kill and a narrow escape.
